Automatic Cursor Placement

When invoking the Barcode Entry form, the cursor will 
appear automatically in the first text box field.

Sorting/Deduping

The 'order by' portion of the SQL query that returns the 
shelf list was adjusted in order to insure accurate deduping
in cases where multiple items have the exact same call number
and no distinquishing item enun/chron/year information.

HTML Cleanup

Cleaned up some sloppy HTML coding.
